Steps

Repair the Eternal Prism (note that you will not be offered the pop-up quests in the instances if you don't have this quest active)
1. Complete the Quests in Miragul's Phylactery: Scion of Ice
2. Complete the Quests in Miragul's Phylactery: The Anathema
  - Necromantic Neverwhere automatically offered when you enter The Crypt of Disdain
  - A Sorcerous Tune automatically offered when you enter The Fundament of Sorcery
3. Complete the Quest in Miragul's Phylactery: The Crucible
  - Stifle the Sentience automatically offered when you enter the zone
4. Use the box at next to Lucretia Nasin in Hollowfrost Grove and receive the Fragment of the Eternal Prism
5. Use the Fragment of the Eternal Prism (can be in any of the Miragul's Phylactery zones)
Return to Connor Farquharson

Steps 4 and 5 can be done at any point during the quest progression, he will have a book over his head when you get out of the Crucible in this case.
