This is initiated by examining a metal bandolier. This item is dropped off random mobs in Enchanted Lands.

Kill fear feeders 36^^^ Heroic around 395, 29, -828 until you receive :a ripped out heart, a rare update...
Harvest 20 Rythmic Stones in the Enchanted Lands
Kill goblins until you find one with some blackened metal (auto updates - all Runnyeye Goblins work)

Once you get that update inspect the bandoleer to complete the quest.
